Please complete this form to register for the Fall 2026 program by August 25th. Upon completion of this form, you will receive more information to help prepare you for your first meeting. Undergraduate cohorts meet on Tuesdays at 3:30-4:45pm and Graduate cohorts meet Tuesdays at 5-6:15pm beginning September 1st.

I understand that as a participant of the Certified Student Leader Program, I must complete the following requirements:

Attending each cohort meeting on time and prepared. Notifying my cohort lead at least 24 hours in advance if unavailable.
Conduct an interview with a leader
Participating in one pre-approved Community Engagement Event or one pre-approved Community Connection Event
Completing a program pre- and post-assessment survey
